Requirements of the Star Council Award:
In order to qualify for Star Council the Council must earn the Columbian Award, Father McGivney Award, and Founder's Award.
Columbian
1. Service Program Personnel Report Form (#365) must be completed and returned to the Supreme Council Department of Fraternal Services by August 1. Report #365 is printed in the Supreme Council's "Council Report Forms Booklet" (#1436).
2. Council must annually conduct and report at least four (4) major involvement programs in each of the following sections of the Service Program: Church activities, community activities, council activities, family activities and youth activities. (Sponsorship of a parish round table fulfills all four Church activity requirements. Sponsorship of a Columbian Squires circle in good standing fulfills all four youth activity requirements.)
The application form to be used in reporting these programs is included in the Supreme Council's "Council Report Forms Booklet" (#1436) included in the "Surge . . . with Service" kit mailed at the start of the fraternal year to every financial secretary of record. Councils not in receipt of the form should contact the Supreme Council Department of Fraternal Services. The completed application must reach the Supreme Council office by June 30.
3. The Annual Survey of Fraternal Activity Report Form (#I 728) must be completed and returned to the Supreme Council Department of Fraternal Services by January 31 . Survey form #1 728 is printed in the Supreme Council's "Council Report Forms Booklet" (#1436).
Father McGivney Award
Service Program Personnel Report Form (#365) must be completed and returned to the Supreme Council Department of Fraternal Services by August I . Report #365 is printed in the Supreme Council's "Council Report Forms Booklet" (#1436).
Achieve membership quota:
Quota for councils with 30-100 members will be an increase of 5 membership additions over deductions based on total active membership of the council as of July 1 . Quota for councils with 101 or more members will be a 5% increase of membership additions over deductions based on the total active membership of the council as of July 1. Minimum of 30 members required. (inactive insurance members are not included when computing the July 1 quota at the Supreme Council office.)
Additions will be credited for new members, juvenile to adult membership, reinstatements, reactivations, readmissions and re applications processed and recorded at the Supreme Council office between July 1 and June 30.
Deductions to be charged against the quota include all suspensions and withdrawals processed and recorded at the Supreme Council office between July 1 and June 30.
Net gain or loss to be affected only by items (b) and (c) in this section. Transfers in or out of the council and deaths will not be counted toward or against the quota. (Monthly computer reports reflecting membership gains and losses will be supplied to each grand knight and financial secretary.)
(Note: New councils must meet the required membership quota. New council quota is based upon the number of members recorded on the Notice of Institution, minimum 30 members required.)
Achievement of the membership quota will be automatically determined at the Supreme Council office through receipt and recording of membership documents (Form #100) as of June 30.
There is no application form to be completed for the Father McGivney Award.
Founders' Award

Quota for councils with 101 or more members will be a 2.5% increase of insurance additions over deductions based on the total active membership of the council as of July 1 . Minimum of 30 members required. (inactive insurance members are not included when computing the July 1 quota at the Supreme Council office.)

Earning the double star should be simple, if you have met the above requirements you are already there, the only requirement is to double the number of new members required for star council.
